"Not able to renew bus pass "

About: Wishaw General Hospital / Ear, Nose & Throat

(as the patient),

I sent back hearing aid report they sent to me. I've had no reply since July 2012. In this letter I have the required hearing loss to renew the bus pass I have had since 2006. However the Glasgow bus station asked for a fax and copy to be sent to them. I was under the numbers and was refused so who does the report belong to and how did I get the bus pass in the first place.
