My father was discharged from Minster ward last week - and have to say that the discharge was not brilliant.
The physiotherapist said he could climb stairs - but clearly hadn't listened to the type of stairs that were in the house.
Neither did they assess whether or not he could stand by himself.
He is very weak and has no upper body strength.
In my opinion he should not have been discharged based on these 2 facts alone.
He then went to the discharge lounge - but nothing was said about the next steps / district nurse etc etc and we were left alone.
My father then needed an ambulance in the evening as he struggled to get up the stairs and nearly collapsed and he was re-admitted to A&E.
The doctors and nurses were lovely - however there was one nurse on Wednesday night who tried to make my father stand on his own.
As he has low blood pressure - this was not a good idea and he started to faint and the Doctor and I had to catch my father to save him falling.
I did ask for this to be noted in his records that I was not happy that this situation had occurred.
