"Quality End of Life Care"

About: Dorset Healthcare University NHS Foundation Trust / End of Life - Palliative Care Swanage Community Hospital

In August of this year my Grandfather was moved to Stanley Purser Ward for rehabilitation with a view to getting him home post a fall, over the weeks that he was on the ward it became more and more apparent that we would not be taking him home. The care he received by all of the staff on Stanley Purser was infallible. He was able to share information with us in such a way that it was clear the nurses and doctors were keeping him fully informed of the developments in his health and involving him in the decision making process, and as necessary/appropriate my father was also involved in these processes. Eventually, in October, it was decided that we would like to take my Grampa home for his final weeks/months. The staff were fully on board and took to ensuring we would be able to do this as soon as possible and with as much support as there could be. Less than a week later, on the day a discharge planning meeting was to take place, we received a call that his health had deteriorated and my parents were advised to go in to the hospital. On this day, we were allowed full access to his room, we were greeted warmly and offered advice in a timely and sensitive manner. We were given the time and space to be with him as he died and the staff caring for him, and us, on that day embodied the "6 C's" - care, compassion, courage, communication, commitment and competence.
