"Reenablement service"

About: Chorley Royal Albert Edward Infirmary

(as the patient),

I was sent home with the Re-enablement team set up 4 times a day and my social worker Anna-Marie also set up the service with Age UK/Age concern for the 6 weeks -  1hour shopping and cleaning service, and although I have rung the service, explained and been told that someone would ring me the same day but no contact has been made.

My neighbours and friends are all elderly and can`t help me apart from the odd couple of items. Now Anna_Marie was appointed to me when I was in Wigan hospital and I don`t know how to contact her to get help.

Can you help please?
