"Poor Discharge ward 53"

About: Victoria Hospital / General Surgery

(as the patient),

Firstly I would like to state that the nursing care I received before and after operation was excellent, however the discharge was the opposite. On day of discharge a nurse entered my room stated that I had to pack my bags and sit in a waiting room while waiting on my drugs. I had to sit for 2 and a half hours on a hard plastic chair while waiting on the medications and a lift home. I was only 2 days post major bowel surgery and was told to rest when needed which I found bizare as how can you rest when being made to sit in a plastic chair? The whole time I felt really uncomfortable with pain which gradually got worse by the time I was able to go home. when I eventually got home I could barely stand and felt ill with the pain. Not once did a nurse come to ask if I was ok. Not once was this discussed on admission that this is what would happen. I feel that this was a contributing factor to me being readmitted the next night after discharge due to severe pain and a high temperature. Also no observations where carried out on my leaving the ward.

I also would have appreciated if I was instructed in wound care as I was provided with no instructions in how to keep my wounds clean and what concerns I should look out for in regards to infection.

I would also like to state that at my initial appointment with the consultant I was made to understand that after operation I would be relatively in little pain and feeling well however it wasn't until I was readmitted when a different consultant informed me that patients with my condition where likely to have high temperature and drowsiness. So when I did feel this way my partner and myself were worried I had an infection.

Thanks so much for giving me a call to discuss your post further. I have been able to discuss the points that you raised with Lorna, the Senior Charge Nurse in Ward 53, she was sorry to read about your discharge experience however she was grateful for your feedback.

Lorna agrees that the chairs in the sitting room are not suitable for patients who, like yourself, are waiting to go home following major surgery. She is in the process of purchasing a couch and more comfortable chairs for this area and these should be in place in the near future.

We appreciate that once the decision has been made that patients are fit to go home, there can be a delay in the patients discharge medication being ready. We are currently working to resolve this with pharmacy, medical and nursing staff.

We had a chat about the information that patients are given on discharge and it is normal practice for patients to be provided with a wound care information sheet. However, I know that this didn’t happen in your case and Lorna will ensure that the discharge checklist is used by her staff as an aide memoire.This would ensure that the patients had everything that they need in place for going home.

I know that the other points you raised will be discussed with your consultant when you come back to clinic.

We are grateful that you have taken the time to post about your experience on Patient Opinion as this has given Lorna and her staff the opportunity to review the discharge process and to make some positive changes in Ward 53.
