"Great Expectations - what a let down."

About: Stepping Hill Hospital

Is it unreasonable of me to expect information pertinent to Mum about her eating and mobility to be written on her notes, instead of the notes of another totally bedridden patient? Is it unreasonable of me to expect my expressed concerns about this to have been passed on, (by the nurse to whom I explained about the notes), to the Charge Nurse, so that when I mentioned it later, it didn't come as news to them? Is it unreasonable of me to expect the charge nurse to know why Mum was suddenly incontinent instead of just brushing me off with 'There's no clinical reason'? (Fortunately the doctor was able to explain the cause of this) Am I unreasonable in expecting that medication to soften faeces and avoid constipation would have been given, especially since it occurred more than once? And, since the NHS website gives detailed information about discharge procedures, is it unreasonable to expect that these will in fact happen? Or should I accept their blithe response that it often doesn't happen that way and that people do leave the ward without a written care plan in place? Since Stepping Hill did not reply to a previous review, I don't expect to hear this time, but by sending the review as questions, I hope it has made it easier for the Hospital to reply.
