Please reply today, your feedback is anonymous & important to us. Texts are free of charge. Thank you. Please reply STOP if you do not want to receive further messages from the trust." Please could you confirm how you expect him to respond when he passed away in your hospital on Tuesday evening. Have you got not sense of respect or dignity or does your drive towards hitting KPIs and performance targets override any sense of common decency.

We are truly sorry for this and can fully understand how distressing this must have been. The issue has been escalated to senior staff and they will be looking into how this happened and what can be done to prevent it from happening to any other family.
