"Lack of communication on my mother's discharge from Leighton Hospital"

My mother, age 87 years, was admitted to Leighton a few days ago on a 999 call having collapsed at her care home. She was still there yesterday on the emergency admissions ward, apparently still having bloods taken and tested.
On two occasions yesterday the family were told she would be staying in for more blood tests. Yesterday evening her care home received a telephone call to say she was being discharged. She arrived by ambulance around 10pm in her nightie with a blanket around her.
Care home staff noticed she had a cannula in her arm for keeping the vein open and she was still wired up to a monitor that was strapped on her. The staff said she had to go back to the hospital to get these removed as they couldn't be responsible.
Back she went to Leighton and then returned to the care home, all late in the evening. She was confused with what was happening.
Is this the way to treat an elderly patient and what happened to the communication between the hospital staff and the family?
