97 year old granddad discharged home. Had requested support with washing/dressing on return home. Expected assessment from hospital discharge team which was cancelled (don't know why), OT refused to order equipment as had already left ward and no one had told him of discharge. Sent home with city Social Services phone number to sort his own care out (he is deaf). Unable to get up out of armchair without support, no pressure care (as had in hospital), not able to stand to prepare meals and unable to carry them through to lounge. Myself/mum slept over 1st 2 nights to get him up and prepare meals/drinks.
Thank goodness for City Care Team, assessed within 2 days and equipment and carer support provided. We feel he would have been re-admitted without this quick response, granddad is now regaining his independence and cooking his own meals. Words cannot express how grateful we are to the community teams, so a very big thank you to Usman and the City Care Team and the GP's/Nurses at Aspley Medical Centre for enabling a very much loved dad, granddad and great granddad to be supported in his own home.
As for city hospital/Southall ward you wonder why people are re-admitted, I feel poor hospital discharges that's why! The pressure you must put on the community teams discharging people like this, you need to take a leaf out of the community teams book as they got everything right and you got it wrong. I am sure you would be very upset if a relative of yours was 'dumped' at home with just a phone number.
