My husband attended the said clinic on 1/10/2014 his appointment time was 2pm, his reason for going is he has a metal on metal hip so needs regular monitoring. On arrival he was told his notes had not arrived at clinic,as this was not the first this has happened he was not very happy, the notes arrived at 2-20pm. He was call in at 2-55pm, then waited a further 15minutes for the member of staff to arrive only to be told the reason for the visit was to have a blood test. With a little organisation the forms could have been sent out so he could have had them done, and the member of staff would have had the results to hand. I was a community midwife before retiring. I ran 2 clinics a week so know how to organise a clinic. The last words were this is how the clnic runs so you will have to accept it.
