"Appointments failure to send letters out."

I was waiting for results to see if had cancer. I should have had a meeting with my Dr in August but no letter sent to me of the appointment. They then booked an appointment in September but again failed to notify me. There excuse, we have a new system and this is happening a few times. What a poor excuse about such serious matters. Someone needs to be taken to account over this because of the stress it causes. I now have a copy of the last letter from Boston hospital and it says I cancelled the 1st appointment even though they did not tell me.
