"Still waiting for my 2014 Appointment letter"

I recently wrote a review on 16 January about my visit to the out- patient in May 2013 when I checked in at the department desk the lady ask if I wanted a letter about the findings of my vist I said yes please,well after waiting a long time I rang my MS nurse on the 12th of May 2014,she did not no why I hadn't recived a letter and when I ask to check about my yearly review letter to my horror she said the appointment had not been made and that she would chase it up.I did get a reply about my first review it to ring the Patient Relations Manger or the Adminstration Manger Neuroscience on a phone number which I did I got through and the lady would pass on the message about me leaving my name because I had wrote the first review anonymous Well it is now the 2nd June 2014 and I still haven't received my yearly review letter I no mistakes are made looks like i will have to ring my MS nurse again. This is the reason I gave low scores.hopefully this will not happen to anyone else.

We would like to apologise that you did not receive a letter following your consultation in May 2013 despite you contacting the Multiple Sclerosis nurse in May 2014 and the administration office as requested. We are unable to investigate the precise details about your clinic appointment as we do not have your full details however if you could please contact Miss Powell Directorate Manager Neurosciences Directorate on 0191 282 5720 she will ensure that your annual review appointment is confirmed. We would wish to apologise that we did not meet your most reasonable of expectations and have highlighted your concerns with all of the administrative team.
