"Poor follow up for Heart/Oesophagus problem"

I have been admitted into A&E and Cardiology on multiple occasions since March and diagnosed with Atrial Fibrillation, Angina and Reflux Disease. I cannot fault the Ambulance staff - they have been fantastic each time. But I cannot say the same about the Hospital staff. The nurses try their best to help and I am not complaining about the basic care, but I am complaining about the follow up procedures. I was fitted with a Holter monitor for a week at home and when I took it back it hadn't made a single successful recording as the machine was broken. I was given a replacement for a fortnight which did work. The initial consultant suddenly left the Trust and I was sent an appointment through with his name despite the fact he had left! The replacement who I don't know his name as he never gave it to me conducted a very short (less than 10 minutes!) appointment where he was completely obnoxious, failed to explain the results of the Holter monitor results and inferred that I was senile and that I wouldn't understand the scans. He shut my notes and said it was not heart related (despite being on 23 heart related tablets a day and having had a heart attack in A&E one night when I waited 8 hours to see a doctor and the heart attack occurred sometime during a period when I didn't even see a nurse for over three hours). This useless consultant then proceeded to book a Echocardigram which I will now have to wait 5-6 weeks for and I will be referred back to my Doctor for treatment. To date, my doctor has not received any letter. For the past two weeks a Matron from the Community Nursing Centre has been visiting me (she was sent by my doctor) and she is unhappy with the after-care I have received and she is organising a second-option in an alternative hospital. This trust seems to have spent millions building an impressive facade but sadly as soon as you get into the hospital you are left severely underwhelmed with a lack of diagnostic skill and an apparent lack of staff.
