"Still haven't learned any lessons from terrible..."

There is a very poor level of communication i.e. my dad had been saying for over a week whilst on the ward that he was concerned that he was getting a discharge but was told this was normal after a catheterisation and not to worry but the day before they released him back home they decided it now might be something to worry about (especially as oneof the main reason for his illness was a severe UTI!) and have taken a swab. He is now home so what if this swab shows he still has an infection, do I trust that he would be made aware? I have zero confidence that this would be the case. To rub salt into the wound dad came home today but they didn’t bother to give him all the equipment that he needed (and that they promised he would be provided for when he was back home) whether this is through ignorance or some other reason I’ve no idea. They even managed to put the wrong discharge date on the form (he came home today but form states he was discharged yesterday). To say that I would never want any of my friends or family to be treated here would be an understatement. I’m sure there are good nurses and docs here but there is a culture that seems to leave both the patient and their families feeling that no one is talking to each other and that the health of those in the hospitals “care” is the worse for it.
