"Transport was very late, causing me additional stress."

About: Cannock Chase Hospital Telford referral and quality service (TRAQS) West Midlands Ambulance Service University NHS Foundation Trust

(as the patient),

As a registered blind person, it was not easy to sort out appointments. Hospital transport not booked for me. No map enclosed for information and insufficient details of doctors name and what I was coming for.

The other hospital sent no medical records, enquiring about further investigations and I was not told to bring these records.

No personal details were held, such as my medication etc, and time took to do this and wasted more time.

Took 6 weeks to get an appointment and this was an urgent referral.

Unfortunately transport was very late, causing me additional stress.
