(as the patient),

I just wanted to thank everyone in the day case admission unit for their amazing care today. 

I was extremely nervous for my treatment today but Mr Grubb, Claire, Ann and all other staff, the lovely porter and the HCSW’s all made me feel more as ease and really looked after me. 
I do apologise for forgetting the other staff nurses name. I hope not to return, but if I do I would be very comfortable in your care xx
