"Excellent rehabilitation support from the hospital team"

(as a carer),

My mum is recovering from a five week stay in hospital  after a significant fall  and is staying with myself for a short time until she feels confident and able to return home. Although medically fit for discharge my mum was very unsteady on her feet, struggling to walk and feeling vulnerable and unsure about her future. 

A referral was made to the Intermediate Care and Enablement Service (Biggart Hospital) team on Friday 29 April 22 for assistance. On the morning of Saturday 30 April 22 physiotherapist Eilidh  visited and assessed mum and developed a support and exercise plan with her. Eilidh made a referral for a wheelchair (which was received on 6 May 22) and other aids which were delivered and installed on Sunday 1 May 22.  Mums exercise plan was supervised by Lisa (very supportive and kind) and this continues. 

Nurse Cunningham visited and assessed mum on 2 May 22 , made appropriate referrals and responded promptly to a further request for advice by undertaking a follow up visit this morning. My mum and I cannot sing these staff praises high enough! all of whom have been polite, courteous, professional and timely but maybe more importantly have listened to mum, developed a care plan with her, demonstrated great knowledge and skills with an empathetic and caring manner

Many thanks for your kind words and review of our Integrated Care Team (ICES). My name is Karen Wilson, I am the Clinical Nurse Manager for In-Patient Care within the South Ayrshire Health and Social Care Partnership although I do not directly line manage this team they do receive professional guidance from myself.

I am sorry to hear your mum suffered such an awful fall but I am pleased to hear she has finally made the next step in her journey, our aim as a Partenrship is to ensure continued seamless care for our residents and this is a perfect example of that. I will pass your kindness onto the team and their direct line manager, and we wish your mum the best for her next steps in her rehabilitation journey.
