My father who is in his mid nineties was discharged from Chesterfield Royal Hospital toward the end of April.
He was sent home without a discharge plan in place. Why was this allowed to happen?
Since then I (his daughter) have been constantly trying to get things sorted for him.
I have never once been asked if I/he needed any help or equipment. Apparently because he already had a stair lift he was sorted!
It is over 2 weeks now since his discharge and to date I am unaware that he has been assigned an OT to assess his needs. The only thing we have been given is a commode, which arrived 2 days ago, not when he needed it, over 2 weeks ago!
I have found this experience very frustrating and totally inadequate to my father's needs.
