Card received from family of patient 

Thanks for the excellent care and support given to our father throughout the final weeks of his life.  The high standard of care provided by yourselves enables us to care for him at home which he wished for.  Thank you.

Thank you for your kind response.

Then acute care at home team aim to support the patient and families during their episode of illness. I am glad to hear that your dad was able to stay at home with his family during the final weeks of his life and that you felt well support by the team. I will pass on your compliments to all the team.
