The Community Matron - Long Term Conditions Team has looked after my friend for some time now. I was my friends carer and related by marriage. Since their partener died my friend came to live with me and my partener.

Initially we only had a downstairs toilet and bath upstairs. In the interim period until we had a ground floor shower fitted, the team helped my friend to get upstairs and bathe - over and above their duties.

The team was exceptionally supportive and attentive to my friends needs and nothing was too much trouble for them. Sadly my friend died, but achieved a last wish to be at home.

The team organised delivery of equipment quickly to support my friend at home, and also removal of same after their death, all very efficient.

They continued to help family members with the issue of the death certificate to ensure a smooth process, which was very helpful to everyone.

I hope if anyone else finds themselves in a similar position as we were, then they are fortunate enough to have the nurse we did
