● Complete lack of commumcation between departments which led to confusion for the patient and relatives. ● When teansferred from one ward to another was told he was not on this pzrticular ward as his name was not on the board. ● Informed staff he had been transferred , they then found he was on their ward in Bay 4. ● Brother comes into visit again informed he was not on this ward, was sent to various other wards ● Although the transfer had taken place over 2 hours ago the board had still not been updated and until he left thd hospital the next day no name above his bed, or on the board. ● Scan arranged was cancelled, unfortunately, this was not told to the patient, who sat waiting fron 12:00 thinking this scan was taking place. This resulted in patient not able to come home until 4:00pm ●Np problem with treatment, but dreadful communication leading to confusion and upset for the patient and relatives.
