My father admitted twice, discharged too soon, excellent care in a& e and cardiac ward but on general wards nursing was variable. Some good staff let down by others mediocre and a few poor. Nurses shouting at tops of their voices at night. Communication with caring relatives poor with honourable exceptions, but worst was discharge planning once my father found to have advanced cancer. Doctor was kind and communicated well. But overall, poor comms, confusion, no apparent handover from onr shift to another, actually given wrong information, no sense of urgency to get him home. Staff have little apparent grasp of mental capacity issues. Since discharge coordination of care has been awful, heads deserve to roll in Brent and Harrow ccg, good staff must despair of trying to give a good service. Assessment and careplanning for fastrack chc neither needs led nor person centred, very superficial assessment of care needs, I could do better myself. Waited all day til8.30pm for his drugs for discharge. My father is a war veteran, has paid his contributions since 1947 and taken little, now he is getting fobbed off with this standard of service when he is helpless to defend himself. Hospice at home is subcontracted bog standard agency care, no care plan.
