(as the patient),

I was born with Renal tubular acidosis from birth and was on peritoneal dialysis. I then got transplanted when I was 3 years old but sadly it wasn't in within 24 hours. For the next 2 years from age 4-5 I was still on peritoneal dialysis then at age 6 I got my second transplant. The transplant lasted over 11 years till I was 11 years old I took limbfoma in my glands and went through cancer treatment. At the age of 14 the limbfoma returned more aggressive and by the age of 16 years old my transplant was damaged due to intense cancer treatment. To this day I am back on dialysis. I have been on it since I was 17. I then became an adult and it's been an OK experience  meeting new and great people along the way. The nurse and staff are wonderful and amazing. Without them I would want to be here anymore.

I am Joan The Clinical sister in the renal unit, who you know well.

Firstly I would like to thank you for taking the time to complete this form and for giving us such positive feedback.

You have been through quite a journey in your life, and I think the way you have approached your condition is a credit to you and your parents.

Your chirpy personality and positive outlook towards life has made our job very easy and it has been a pleasure looking after you over the years and hopefully forthcoming years.

As you know we would be interested in hearing any ideas that you would have to enhance patients care during their dialysis session.

I will ensure to tell all the staff of your very positive feedback.

I would take this opportunity to thank you and your parents for the fund raising events that you have undertaken over the last few years for renal patient comfort.
