"The care and attention we have received has been second to none"

About: Intermediate and Transitional Care / Community Discharge Service

(as a relative),

After discharge from hospital my mum had a variety of needs to be met in order to allow her to come home. The care and attention we have received has been second to none.

Our situation is not particularly straightforward and the level of understanding and willingness to help from the team has been invaluable.

The most gratifying thing is that they listened and took a holistic approach. They understood that this was new territory for us and guided us all very well.

They also did what they said they were doing to do when they said they would do it, without being chased up…that is very rare these days in my experience…so again…invaluable.

Our sincere thanks and best wishes to the team.

Thank you so much for your kind words regarding the Community discharge Service and the team members that attended you after getting home from hospital. I will pass this on and they will be delighted to hear this.

I am delighted that you felt the team listened, understood, were willing to help and followed up on plans. We pride ourselves in trying to meet the many complex needs of people post discharge, which can be different for everyone and it is lovely to hear they have made a positive impact on your journey to recovery.

Wishing you well and continued improvement.
