About: Community services / Falls Prevention Service

(as a relative),

- Laura visited my home following discharge from hospital.

- Very detailed visit and covered all aspects of my health care and mobility.

- Her attributes: Pleasant / put me at ease / interested in all aspects of my         health and care / knowledgeable / patient.

- Carried out physical checks also - walking unaided / walking with walking     frame / checked out bathroom facilities.

- Took blood pressure and heart rate in different positions etc.

- Left leaflet re Home Safety check

- Laura to refer me to community OT for bed and walking assistant. District     nurse. Incontinence team for urgent review. Social work for shower        assessment.

- Also advised to contact my GP on other health issues
