I had ongoing mental health issues and was with CAMHS from a young age. None of the people assigned to me worked.
In my mid teens I was referred to head 2 head as I was excluded from school due to my psychosis. I had Sarah as my care coordinator for 2 years. She was amazing and made me care plans that saved my life. She made me feel like a human, not just a patient. The people before her made me feel that way (not head 2 head). She made me feel like a friend. I felt like I could tell her anything and she wouldn’t judge me. She met the people closest to me and allowed me to explain to them why I was the way I was on my terms.
I got discharged a month a go. I still have bad days, as everyone does, but the safety and plan put in place makes me know what to do in those situations and everyone around me notices a difference.
Me and my family couldn’t be more grateful for the help she and Phil (the man who assigned Sarah as my care coordinator) provided.
