"Support from my stroke"

About: Older People Community Services / Community Stroke Team

(as a service user),

I had a brain haemorrhage and couldn't praise the community stroke team and nurses enough.

I was realised and a community stroke nurse was there that evening to reassure me and inform me of what happens now about my recovery at home which I was not really told about in hospital.

They helped me multiple times per day with care needs and how to carry out and re learn simple tasks. My recovery would not have been as quick without them. 

Donna, Stephen, Ronan also visited multiple times before I was discharged and again could not thank them enough and praise them highly enough for the help and assistance in my recovery

It is heart-warming to hear you had such a positive experience with the community Stroke team.

We aim to provide a seamless service and offer support and reassurance when you leave hospital and move home - we are delighted you were happy with our service

I apologise that from the acute setting that the next step in your recovery was not explained clearly, I will endeavour to feed this back to the team and ensure that in future we inform patients of all relevant information on discharge.

I am delighted to hear your positive experience with our community stroke team, we are very proud of the service they deliver. Roseann has mentioned above she will ensure this is feedback to the individuals mentioned.
