"Premature discharge, lack of care"

I have been attending this clinic since early 2020. I have yet to receive the help I need or have requested. I made a complaint about my care years prior, during the pandemic. I was promised that more would be done to help me. This did not happen.

My previous consultant, left the practice without notice (I was informed that they had left, and it took almost a year to replace them). I had a single consultation a new doctor at the practice. I spent the first half expressing dissapointment and asking questions, as I was frustrated that I was seeing new doctors every appointment. They told me they were a permanent doctor but I wouldn't be seeing them everytime.

Come the end of our appointment we had agreed on treatment, but two weeks later I found out I was discharged. I have the entire consultation recorded, so I can reference this, and use it as supporting evidence.

I have some serious concerns about the Dr treating me and would like someone from Eastvale to reach out to me. My treatment has been stopped when I need it most and things have been told about me that are untrue and the Dr. cannot possibly verify within a single 30 minute appointment (especially considering I was frustrated and talking meta (i.e. my care and how it's provided)).

I have made many attempts to rectify what I thought was once a misunderstanding. Eastvale have made it clear that they are unwilling to consider any option and have told me to complain.
