"Lack of communication and DNR"

About: University Hospital Monklands

(as a service user),

I was treated in the hospital recently and was shocked to find a do not attempt cardiopulmonary resuscitation (DNACPR) form in my bag of discharge medication. This was not discussed with either myself or my family. Furthermore, the information on the form itself is sparse e.g. the role and designation of the person who signed the form is not completed, an explanation is limited only to see notes, and it is dated the day after my discharge.

 As I am sure you will appreciate, this is a highly distressing situation and I would appreciate a full and detailed explanation. I would anticipate that a review of process would be initiated to avoid another patient having a poor experience as result of the current practices being used in Monklands.

Thank you for sharing your experience on Care Opinion.

I am sorry to hear of the shock you received seeing the DNACPR documentation in your bag on discharge. I can understand your distress at this, especially as the information on the form is sparse or missing and that no one had discussed this with you or your family at the time. It also seems very unusual that the form is dated the day after you were discharged.
