"Charges for medical forms"

My daughter needed a medical form signed for her student application for camp America.  She handed it 2 weeks before Christmas and was told it could take up to 8 weeks to complete and what the charege would be, and that when she came to collect it she would be told exactly how much it would be. We understood there would be a charge and know that it is up to individual doctors/practices as to what they charge but was shocked at how much they were charging.  She is a university student still in full time education and after doing some general research the general consensus of what the charge should be was only half of what hey were charging. 

They also gave her a form of authority to authorise me to collect the form if it hadn't been done before she had to go back to university. 

We took this form in on Monday to the reception and my daughter was then going to pay even though it hadn't been done. We were only then told they only accept cash and as my daughter hadn't got any they directed her to the nearest cash point.

At this point I questioned this as being very strange for a medical practice but they said it was their policy - cash only! The charge was confirmed and I took cash out of my purse. We were then told it had to be EXACT cash as they didn't keep cash to give change and if I wanted to go to the chemist next door to buy a bottle of water and get change so I could pay.

To say I am astounded at a medical practice having this policy is an understatement.  Fair enough have a cash-only policy but surely you then have a petty cash where it goes so you would then have change to offer if it was your policy.

Not only do I feel the fee is very excessive, I felt that the way we were asked to pay was very unusual. 

I have tried to find an email address or contact details for a practice manager but can't see one on the website. 

I know its only a minor issue but it doesn't feel right and I wanted some other people's opinions.
