"End of life care"

About: Community Nursing / District Nursing General practices in Forth Valley

(as a relative),

My mum passed away earlier this month from Alzheimer's disease. We required end of life care at home from the District Nurses for the last 5 days. They did this with care and compassion and in a professional, organised manner. Not only did they care for mum but as her daughter I felt supported and cared for too.

First of all, please accept my sincere condolences for the loss of you Mum. Thank you for this lovely feedback given at such a difficult time for you, it is much appreciated and really kind of you to take the time to share this with us. I will pass your thanks on to the District Nurses.

As a clinical Service Manger it is always valuable to receive feedback on the service we provide, so I would like to thank you for taking the time to complete this care opinion survey.
