My elderly father had a stroke this year and was discharged home in August. He received excellent rehab at home from START (Stroke Transition and Rehabilitation Team. Jennifer (Physio) and Simone (therapy assistant practitioner) visited him mostly daily through the week and worked with dad on his specific goals, achieving and excelling many of them. 

They took a person centred approach and gave dad confidence to try new things and to keep progressing to the next objective. We also had input from the OT's and SLT which altogether showed excellent Multi Disciplinary Team (MDT) working and communication. Thank you all.
