"Mental health provision"

About: Cumbernauld Locality Support Service / Community mental health team outreach

(as a service user),

Despite it being a legal requirement to create a care plan for each patient and share it with them I have been refused access to my care plan by Cumbernauld CMHT.

It's bad enough they have completely dropped their duty of care over my mental health without having to fight to see my care plan

Thank you for sharing your concerns with us. The reason you were refused access to your care plan was the nurse wanted to be given the opportunity to discuss your care plan with you.

The purpose of a care plan is for shared decision-making to achieve joint goal setting by the clinician and the patient

Dear unhappy

I am sorry that you have had difficulty accessing your care plan. To support development and agreement of a care plan we always try to do this in collaboration with you. This allows the final version to be discussed, agreed and shared. If you contact your CPN I am sure that they will arrange an appointment to facilitate this.
