"Given the wrong form"

About: Tameside General Hospital / Maternity care

(as a parent/guardian),

My daughter visited the maternity ward as a pregnant outpatient.

I felt very worried and concerned that she was given a form for a blood transfusion by a midwife, instead of a form for bloods to be taken to check for anaemia.
