About: Telford referral and quality service (TRAQS)

(as a service user),

At 2 yearly sight test at opticians, I was advised referral would be made to GP re: cataract, left eye.

Secretary at GP practice rang advising me height, weight and blood pressure needed to be checked before referral to eye clinic could be made.

Health care assistant did this, said secretary would now make referral.

At no time did I see GP or have ANY information about referral/procedure.

Thank you for taking the time to provide feedback to the TRAQS team.

I am sorry that you were not informed that your referral would be made via TRAQS and that the team would be contacting you to offer Choice and to arrange an appointment.

TRAQS have provided all GP practices with a supply of leaflets which should be given to patients being referred on for other treatment to explain how TRAQS works and what will happen next.

Please accept my apologies on behalf of your practice that this did not happen but thank you again for responding with feedback to the TRAQS team.
