"Problems with discharge and aftercare"

Ward C32 I was not given a discharge letter. I had a really awful night as I had a reaction to the antibiotics I was given. I did not realise this at the time and called the hospital ward 3 times during the night. They were not very helpful. They just advised going to a and e or my GP because the symptoms were not related to the procedure that I had. I went to my GP the next morning and because I had not come home with a letter from the hospital and because the doctors surgery had not received anything, it took my GP longer to see me. He also tried to call the ward and after waiting quite some time on the phone, he had not got through and by then I was able to give him enough information.
Not being given a letter on discharge, having a worrying night of pain, my GP not having any information from the hospital is not acceptable.
