About: Locala Community Partnerships CIC / Heart failure team

(as the patient),

I have been diagnosed with heart failure resulting in fatigue and kidney complications.

I have now been visited on numerous occasions by our local heart failure nurse Anne Malloy (Kirklees NHS).

She is caring, attentive, gives good advise and gives me the confidence to cope with this condition.
