It is a great hospital with a wonderful reputation but unfortunately, in my opinion, based on long experience,. it is losing its way because it is burying itself in a paperwork, form filling quality system. As so often happens in many industries, there is a very real risk of ticking the right boxes becoming more important than the patient.

Paper driven systems must not replace nursing care!

On the 4th white page there is an important note, but the nurse doesn't see it because she is busy filling in the 3rd green page. If the reader doesn't recognize these colours I suggest they spend more time on the wards.
