"Nearly given incorrect drug"

I'm on a 2 monthly cycle hospital visit to receive a drug.

Over this and last year, it has always been the same drug.

On yesterdays visit, the drug that was going to be given was correctly documented and correctly prescribed, but a different and incorrect drug was prepared!
