90 year old patient admitted following fall, no liason with family during hospital stay, no liason over discharge with family - all despite requests from family and concerns being raised by family.

within 2 hours of being discharged elderly patient had fallen again suffering serios injury that will likely impair her mobility for life. it will certainly impair her quality of life

We are really sorry and disappointed to read your comments about the Royal Berkshire Hospital with regard to the lack of communication between the ward staff and the patient’s family, and the anxiety that was caused to the family. We strive to provide the best possible care and ensure that the patient’s journey from admission to discharge is a seamless one. This includes involving significant relatives so that any concerns they may have with regard to discharge planning are addressed accordingly. Unfortunately, this does not reflect your experience for which we sincerely apologise. Because you don’t identify the department or ward it is difficult for us to investigate and know where to focus resources to ensure this patient’s experience is not repeated.
