"Lack of communication at Barnsley District General"

In total, my elderly father has been in the Barnsley District General hospital for 7 weeks. He was originally admitted with a suspected stroke, but was later diagnosed with a water infection and a chest infection. Due to this, he has been unable to walk unaided and has been very confused. AS he got better, he was sent to a rehabilitation unit. This happened very quickly. The previous day we were told he needed to be assessed and that a meeting would be called with the family to discuss his needs, then suddenly the next day he was moved.

He was at the rehab unit for 2 and a half days before becoming very poorly again, and so was readmitted back to the hospital. We believe that his water infection was not properly treated in the first place.

There has been a lack of communication between the medical staff and our family as to what is happening regarding his rehabilitation. Someone spends 5 minutes assessing him and decides what's best. How can a 5 minute assessment, without a family member present, give a true picture of how well a person is?
