Its not the first time I have reported this problem. On this occasion both nurses and social services are updating the notes of the patient with the actions that need to be done, but neither pay any attention to those of the other party. End result patient discharged before action of social services are carried out, and absolutely no continuity to the next stage... info does not go with patient.

Also, patient as usual receives too little movement/physio activities when that is what is needed. Then goes on to rehab place where they totally give the wrong treatment due to lack of information and insisting on doing their own thing because they know best. In this case Springfield rehab... Do not go there for rehab.
