"appointment lost in the post"

About: Telford referral and quality service (TRAQS)

(as the patient),

1: I was contacted by phone saying that TRAQS had taken over my physio appointment and would I fill in the form, which was most important.

2: 26th September, received a letter stating I should not contact them.

3: Towards the end of october I phoned them to see if my first physio appointment was anywhere near due. They stated that my appointment must have got lost in the post but was told madelely physio centre would contact me, which they did very soon after.

4: I suspect that I would still have been waiting for TRAQS to contact me if I had not have contacted them
