Recently my mother was admitted to hospital and between leaving the care home and arriving at her final ward her teeth and spectacles went missing. There was no inventory kept by the receiving ward and by the time she reached Ward 15 the inventory showed neither teeth nor spectacles.

A very worrying lack of robust organisation.
