"Wasn't sent personal details form prior to Princess Royal appointment"

About: Telford referral and quality service (TRAQS) The Princess Royal Hospital / Ophthalmology

(as the patient),

I did not see my GP as I gave GP a letter from my optician for referral. I was asked at Princess Royal Hospital for my personal details form, re. religion/DOB/creed, but had not been sent one before my appointment date in November.

Thank you taking the time to reply following your referral by your optician via your GP and TRAQS.

I note your comments with concern and apologise that you did not receive the form that was requested upon your arrival at the hospital. However, the form you mention is sent out to patients by the hospital not TRAQS.

I hope that your treatment is going well and thank you again for the time you took to provide feedback to TRAQS
