"Cannot get treatment plan for DVT"

My wife was diagnosed at St Thomas Hospital with a DVT soon after giving birth to my daughter. The hospital sent us home with 6 weeks worth of Clexane injections and the promise of a treatment plan for the next 6 months consisting of follow up appointments, tests and different drugs.

After a week we contacted the hospital to try and find out when we would get our next appointment and treatment plan. They said we should contact our GP. We then went to the GP and he said we should contact the hospital.

We insisted on the GP following up and he promised to chase up the hospital and come back in a couple of days. When we contacted the GP again he said he had no success and suggested my wife come in so they can call together and my wife can guide the call. When we did that the hospital said they would have someone contact us. They never did.

When we called the hospital again we were told that if "the DVT is worrying us" we should go to A & E. Of course the DVT is worrying us, but we've already done that stage, we need our treatment plan.

It has been 4 weeks of this absolute bureaucratic nightmare. We're desperate the get my wife the treatment plan she deserves, so she has a higher chance of surviving what is potentially a dangerous condition. What can we do? Who can we call? Is there some sort of NHS escalation hotline?

Where is this 1st class healthcare system the UK boasts about? Ever since my wife got pregnant, we've gone through the system and it is shocking at every step how much goes wrong.
