"My brother's death from pneumonia"

My brother had been unwell. He had a GP appointment but on the way he collapsed.

An ambulance was called he was taken to the local A&E department. He was admitted with pneumonia. After 2 days he was discharged with antibiotics and steroids to take. He was told a follow up x-ray to be done in 6 weeks.

He finished his tabs and the second week he became unwell again. He looked rough, sweats, trouble breathing etc. We called our GP who visited him at 2:30 on a Monday to be told the pneumonia was still there as they heard a little crackle on his right lung. They gave him more antibiotics and said if you get worse go to hospital.

Two hours later we found him dead in the living room. The post mortem showed severe pneumonia with pus on his lungs. How did the doctor not see this and why did the hospital discharge him after 2 days, and a follow up was not done sooner?

Our family are in shock, as my brother had no other major health issues. Something is wrong somewhere.
