What could be improved

Huge huge communication flaw between consultants, nurses, and informing family members of action being taken without even bothering to speak to the family, DNR put in place without any communication to the family had I not happened to open the file and stumble across it I would never have known, I did however subsequently have it removed after much passing of the buck beetween each other also was signed up for a drug then told it was never administered, I have photographic proof it was given and signed as given along with the dose given, and the date given this will not be the end of this complaint by a long shot this is just the beginning
