About: Telford referral and quality service (TRAQS)

(as the patient),

My husband is also my carer. I have had arthritis for many years- its in my hands and back- I. e. my hips and shoulders.

It would be good to get some help as I'm not too good at taking pain killers.

My husband completed this form for me as I am partially sighted.
