What I liked

This feedback follows a stay by an elderly family member and I appreciated the efforts being made by the staff to keep me informed.

What could be improved

Both at the end of this stay and a stay in December 2011 I felt the discharge process was unacceptable. There seemed a total lack of co-ordination between the ward , the pharmacy and the transport team. An elderly person can easily become confused and upset so once the decision is made to discharge, the actual day of discharge should be a formality and it should not be acceptable to keep the elderly person sitting around all day. In both instances my relative was eventually discharged in the evening. Because her day was so unsettled she had not had anything substantial to east or drink for several hours. The staff are no doubt committed to providing good care but the communication between departments seems poor. Is there any measure of discharge performance?

After both stays it has taken my relative a few days to recover from the discharge
